What you are buying

A digital file — an Excel workbook (.xlsx) or a PDF — delivered as an instant download. No physical goods are shipped.

Licence

One purchase gives you a perpetual licence to use the file inside your own organisation, on any number of your own devices, and to adapt it for your own records. You may not resell, redistribute, publish or include the file (or a lightly modified copy) in a product you sell.

Price and payment

Prices are shown on each product card and are one-time payments in US dollars. Payments are processed by Creem (merchant of record) or Ko-fi. Taxes are handled by the payment provider.

Refunds

Because delivery is instant, sales are final once the file has been downloaded. If a file will not open, is incomplete, or is not the version described, contact support@wuplab.com and we will send a corrected file free of charge — or refund the purchase if we cannot fix it.

No professional advice

These are blank record-keeping and workflow templates. They do not contain medical advice, diagnosis or treatment guidance, and they are not a certification of compliance with any regulation. You remain responsible for the accuracy of the data you enter and for meeting the requirements that apply to your facility.

Limitation of liability

The files are provided "as is". To the extent permitted by law, our liability for any claim relating to a purchase is limited to the amount you paid for that product.
